4) Partnership & Agreements: When applicable, we complete any required agreement, referral process, MOU, or
service documentation.
5) Schedule Services: We coordinate dates, logistics, and patient referrals.
6) Provide Care & Follow-Up: We deliver care and coordinate follow-up recommendations as needed.

4) Consent & Required Documentation: We provide and coordinate required parent/guardian consent forms, health
information, and other documentation needed before services are provided.
5) Schedule School Services: Once planning and documentation are complete, we coordinate service dates and
communicate what families and school staff can expect.
6) Provide Care & Follow-Up: We provide preventive dental hygiene services and oral health education in a
comfortable, student-centered environment, with follow-up recommendations or referrals when appropriate.
