1) Request a Visit: Email smileofhopedhp@gmail.com or submit a request below for mobile dental hygiene services for yourself, your family, or your organization.​

 

2) Registration Packet: We will send you a registration packet, which includes a welcome packet, health history forms, and consent forms. Please complete and return the forms at your earliest convenience.​

 

3) Clinical Review & Care Planning: A dentist will review the health history and determine whether additional information is needed from the patient’s primary care provider. ​ The dentist may request and review any X-rays or intraoral photos and will recommend a treatment plan based on the patient’s needs.​ Preventive services will be provided by a Registered Dental Hygienist in Alternative Practice (RDHAP) within their professional scope of practice.​

 

4) Schedule Your Visit: After we receive your completed registration packet, we will reach out to you to schedule your visit at a time that works best for you. Please let me know if you have any questions. We look forward to caring for you.​

 

5) Referral: Any treatment beyond dental hygiene services may be completed by a licensed dentist on-site or through referral under their business entity.
